As school routines resume and families spend more time indoors, coughs, congestion, sore throats and fevers can spread quickly. Influenza, COVID-19, RSV and the common cold can look very similar at first. Symptoms alone often cannot identify which virus is causing an illness.
Knowing when testing may help—and which warning signs need emergency care—can make the next step clearer.
Flu, COVID-19 and RSV can all cause respiratory symptoms. Common symptoms include:
RSV often begins with cold-like symptoms such as congestion, coughing, sneezing and fever. Wheezing can occur, and very young infants may show irritability, reduced activity or breathing difficulty instead of the usual symptoms.
Flu often begins more suddenly and may cause prominent fever, chills, body aches, headache and fatigue. However, not everyone with flu has a fever. COVID-19 can also cause many of the same symptoms. Because the overlap is substantial, a symptom checklist cannot reliably confirm the cause.
Testing can help distinguish among respiratory viruses and guide decisions about treatment and protecting others. It may be especially useful when:
Rapid tests are useful, but a negative result does not always rule out infection. Test timing and the type of test matter. A medical provider can review your symptoms, examination and test results together rather than relying on one result alone.
Prescription antiviral treatments are available for flu and COVID-19. They are most helpful when started early, particularly for people at higher risk of severe illness. CDC guidance notes that flu treatment works best when started within about two days of symptom onset, while COVID-19 treatments generally must begin within five to seven days, depending on the medication.
Antibiotics do not treat flu, COVID-19, RSV or the common cold. They are used for bacterial infections when medically appropriate. Testing and an examination can help avoid unnecessary antibiotics while identifying patients who may benefit from other treatment.
For many mild respiratory illnesses, supportive care includes rest, fluids and age-appropriate over-the-counter medicine when safe. Ask a healthcare professional or pharmacist before giving cough or cold medicine to a child or combining medicines.
To reduce spread:
CDC advises staying home until symptoms are improving overall and you have been fever-free for at least 24 hours without fever-reducing medicine. Use added precautions around others for the next five days.
An urgent care visit may be appropriate for non-emergency symptoms such as persistent fever, worsening cough, sore throat, congestion, wheezing without severe breathing difficulty, dehydration concerns, or symptoms that improve and then return.
At 209 Urgent Care, a visit may include a medical history, physical examination, appropriate flu, RSV, strep or COVID-19 testing, review of available results and treatment recommendations when medically indicated. Walk-ins are welcome for children and adults.
Call 911 or go to the emergency department for severe or life-threatening symptoms.

This list is not complete. Seek emergency care for any severe or concerning symptom.
If you or your child has non-emergency respiratory symptoms, 209 Urgent Care provides walk-in evaluation and medically appropriate flu, RSV, strep and COVID-19 testing for children and adults.
